Hiqa inspected Damien House in Co Tipperary on 16 April.
Hiqa’s inspectors reviewed 26 files and found that in 19 of them allegations had not been appropriately notified to gardaí by social workers.
The findings were made as part of an unannounced inspection by HIQA in July.
Fresh Start Foster Care Service said it was ceasing operations due to “ongoing difficulties in relation to retention and recruitment of staff”.
Hiqa said that a detailed plan for ambulance services across Dublin didn’t exist.
Staff at St Raphael’s, Youghal, Co Cork, said there was a lack of staff available at that time to assist the resident out of bed.
The figures are contained in a Hiqa annual report.
Hiqa has raised concerns about hygiene standards after an unannounced inspection.
